How check up is done in piles fistula fissure diseases by your doctor?

There are many diseases is that occur in ano-rectal area, some of them are; Piles or Hemorrhoids, Fissure in ano or anal fissure, Fistula in ano or anal fistula, Perianal abscess etc.
Symptoms of Piles/Hemorrhoids, Fissure, Fistula
In piles patients usually suffer from bleeding during the act of defecation. This bleeding can be in the form of drops or sometimes as splash in the toilet pan in large amount. Bleeding in hemorrhoids is usually painless.
In fissure in ano disease the main symptom is anal pain. The pain occurs during the act of defecation and remains hours after defecation. The pain is usually severe and there may be a small amount of bleeding as streak over hard stool.
Constipation is common symptom in Piles as well as in fissure in ano.
In perianal abscess there may be swelling with pain which is is filled with pus while in Fistula in ano, there may be intermittent first discharge from an opening situated in perianal area with pain off and on.
Why there is need of checkup in ano-rectal diseases?
Check-up by a specialist doctor is necessary if you are having any symptoms of ano-rectal disease because sometimes these symptoms may be of serious disease like cancer and if you visit to a right doctor at right time you can get the the proper treatment after being diagnosed by the doctor.
How your doctor do and ano-rectal examination?
Local ano-rectal examination
First of all your doctor will ask you to lie down in specific position like lithotomy position or left lateral position. Then he/she observes your ano-rectal area with the help of focus light for any ulceration or Fissure or External pile mass or Tag or any swelling or abscess or any pus discharging fistula external opening. Then your doctor may ask you to strain like you do during defecation. Internal hemorrhoids of second grade and third grade prolapsed out during straining.
Degree or Grades of Hemorrhoids/Piles
Digital rectal examination/DRE or Per-rectal examination or PR examination
in digital rectal examination your doctor use his gloved lubricated finger index finger and will gently insert inside of your anal canal and rectum and feel the inside of anal canal and rectum. This helps in diagnosing internal opening fistula in ano, piles, rectal polyps, abnormal growths or mass and prostate problems in males.
Proctoscopy
If your doctor feel necessary, he/she may use a tube like instrument known as proctoscope to visualise directly inside of rectum and anal canal. Internal hemorrhoids of any grade can be diagnosed easily by proctoscopy.
Digital rectal examination and proctoscopy are done with utmost gentleness by your doctor with adequate lubrication by using a local anaesthetic jelly like lignocaine jelly, but if if any patient is feeling discomfort or pain during these check-ups, your doctor may defer or avoid these procedures.
Other ways to examine ano-rectal diseases
Sometimes it is difficult or impossible to diagnose a ano-rectal disease by the above methods of examination. In such case some other methods of examination like Sigmoidoscopy or Colonoscopy or Trans-rectal ultrasonography or MRI pelvis are advised by your doctor.
